B. Current blood pressure readings?

C. How long has proposed insured been on present
     medication?

D. Has proposed insured ever had chest pains? If yes,
     the date of last episode?

E. Has proposed insured had an EKG or stress test done
    since being diagnosed as Hypertensive? If so, what were
    the results and date?

F. Answer ALL questions listed under
    #1 General Risk Profile 

LIVER DISORDERS! HEPATITIS

Definition: The cells of the liver may be injured by exposure to viruses, drugs, alcohol, and toxins. When the cells are injured, abnormal concentrations of certain enzymes may occur

A. Does proposed insured have any history of alcoholism
     or alcohol abuse? If yes, answer questions listed under
     alcohol abuse. (# 2 of Impairment Index)

B. If proposed insured has a liver disorder, indicate type.
     (ie: hepatitis, jaundice, fatty liver, cirrhosis,
      hemochromatosis, abnormal liver function tests) and
      date of diagnosis?

C. If hepatitis, what type... A, B or C?

D. Indicate type(s) and date(s) of diagnostic tests performed
     on your client? (ie: bloodwork, CAT scan, Sonagram,
      biopsy)

E. What are proposed insured's most recent liver enzyme
      readings? SGOT, SGPT, GGPT?

F. Answer ALL questions listed under
     #1 General Risk Profile
